NAD+ Therapy and Cancer Risk: What Prescribers Need to Know

NAD+ plays a central role in DNA repair, cellular energy metabolism, and cellular proliferation. These same functions that make NAD+ attractive for longevity medicine raise theoretical questions about cancer risk: could NAD+ supplementation fuel cancer cell growth?

This is one of the most important questions prescribers need to address when counseling patients about NAD+ therapy. Here is what the evidence shows.

The Theoretical Concern

The concern about NAD+ and cancer is based on several observations:

Cancer cells are metabolically active: Cancer cells have high energy demands and often upregulate NAD+ biosynthesis pathways to support their metabolic needs.

PARP activation: PARP enzymes, which use NAD+ as a substrate, are involved in DNA repair. Some cancer cells rely on PARP activity for survival. (This is actually the basis for PARP inhibitor cancer drugs β€” blocking PARP kills certain cancer cells.)

Sirtuin activity: Sirtuins, which require NAD+, have complex roles in cancer β€” they can be tumor suppressive in some contexts and tumor promoting in others.

CD38 and immune function: CD38, which consumes NAD+, plays a role in immune function. The relationship between NAD+ levels and anti-tumor immunity is complex.

What the Evidence Actually Shows

Animal Studies

Animal studies on NAD+ supplementation and cancer have produced mixed results:

Tumor growth studies: Some studies have shown that NAD+ supplementation can accelerate the growth of established tumors in animal models. A 2019 study in Nature showed that NAD+ precursor supplementation accelerated the growth of certain breast cancer cell lines in mice.

Protective studies: Other studies have shown that NAD+ supplementation can have anti-tumor effects, particularly through enhancement of immune function and DNA repair.

Context dependence: The effect of NAD+ on cancer appears to be highly context-dependent β€” the type of cancer, the stage, and the specific metabolic characteristics of the tumor all matter.

Human Studies

Human data on NAD+ supplementation and cancer risk is very limited. There are no randomized controlled trials examining cancer incidence in patients receiving NAD+ supplementation.

Epidemiological data on niacin (a NAD+ precursor) and cancer is mixed β€” some studies suggest protective effects, others show no effect.

The Bottom Line

The current evidence does not establish that NAD+ supplementation causes cancer in humans. However, the theoretical concerns are real enough that prescribers should:

  1. Screen for active malignancy before starting NAD+ therapy
  2. Avoid NAD+ therapy in patients with active cancer (unless under oncology supervision)
  3. Counsel patients about the theoretical concern and the current state of evidence
  4. Monitor for any signs of malignancy during NAD+ therapy

Contraindications and Precautions

Absolute Contraindication

Active malignancy: NAD+ therapy should not be used in patients with active cancer outside of an oncology-supervised research context.

Relative Contraindications / Precautions

History of cancer: Patients with a history of cancer should discuss NAD+ therapy with their oncologist before starting.

High cancer risk: Patients with BRCA mutations or other high-risk conditions should discuss NAD+ therapy with their oncologist.

The Oncology Perspective

Some oncologists are exploring NAD+ in the context of cancer treatment β€” both as a potential adjunct to chemotherapy and as a supportive care intervention for cancer-related fatigue. This is an active area of research.

However, this research is being conducted in controlled settings with appropriate oversight. It does not mean that NAD+ supplementation is safe for all cancer patients.

How to Counsel Patients

For patients asking about NAD+ therapy:

For patients without cancer history: "NAD+ therapy is generally considered safe for people without cancer. There is a theoretical concern about cancer risk based on NAD+'s role in cellular metabolism, but human studies have not shown an increased cancer risk. We'll screen you for any cancer risk factors before starting."

For patients with cancer history: "Given your history of cancer, I'd recommend discussing NAD+ therapy with your oncologist before we proceed. There are theoretical concerns about NAD+ and cancer cell growth that we want to make sure are addressed in your specific situation."

For all patients: "If you develop any new symptoms during NAD+ therapy β€” unexplained weight loss, fatigue, lumps, or other concerning symptoms β€” let me know immediately."
