If you have ever received a compounded GLP-1 preparation packed in an insulated box with ice packs, you have experienced cold-chain shipping. But what exactly is the cold chain, why does it matter for GLP-1 medications specifically, and what should your clinic be verifying to ensure the preparations you receive have been properly handled?

What Is the Cold Chain?
The "cold chain" refers to the unbroken series of temperature-controlled steps that a temperature-sensitive product travels through from manufacturer to end user. For pharmaceutical products, the cold chain typically encompasses:
- Manufacturing and storage at the compounding pharmacy or outsourcing facility
- Packaging in validated temperature-controlled shipping containers
- Transit via carrier (typically overnight or 2-day air for pharmaceutical products)
- Receiving and storage at the clinic or pharmacy
A "break" in the cold chain β any point where the product is exposed to temperatures outside its labeled storage range β can compromise the product's quality, potency, and safety, even if the product looks completely normal afterward.
Why GLP-1 Preparations Require Cold-Chain Shipping
Compounded GLP-1 preparations β including semaglutide and tirzepatide formulations β are sterile injectable peptides. They require cold-chain shipping for several reasons:
Peptide stability: GLP-1 receptor agonists are peptide molecules that are susceptible to degradation at elevated temperatures. Heat accelerates hydrolysis (water-mediated bond cleavage), oxidation, and aggregation β all of which reduce potency and can generate potentially harmful degradation products.
Sterility maintenance: While the sealed vial maintains sterility regardless of temperature, elevated temperatures can stress the container-closure system and, in extreme cases, affect the integrity of the seal.
BUD validity: The beyond-use date assigned by the compounding pharmacy is only valid if the preparation has been stored within the labeled temperature range throughout its life. A temperature excursion during shipping can invalidate the BUD even before the preparation reaches your clinic.
Regulatory requirement: USP <797> and FDA cGMP standards require that compounded sterile preparations be stored and transported under conditions that maintain their quality. Cold-chain shipping is part of meeting this requirement.
How Cold-Chain Shipping Works
A properly executed cold-chain shipment involves several components:
Validated packaging: The shipping container (typically an insulated box with phase-change materials like gel packs or dry ice) must be validated to maintain the required temperature range for the expected transit time and ambient temperature conditions. Validation involves testing the packaging system under worst-case conditions to confirm it performs as intended.
Temperature monitoring: Most pharmaceutical cold-chain shipments include a temperature monitor β either a data logger that records temperature continuously throughout transit, or a time-temperature indicator (TTI) that changes color if the temperature exceeds a threshold. These monitors provide evidence that the cold chain was maintained during shipping.
Carrier selection: Not all carriers offer the same level of temperature control. Pharmaceutical-grade cold-chain shipping typically uses overnight or 2-day air services with carriers that have pharmaceutical shipping programs.
Timing: Shipments are typically scheduled to avoid weekends and holidays, when delays are more likely. Some pharmacies will not ship on Fridays to avoid weekend delays.
What Clinics Should Verify
When receiving cold-chain shipments of compounded GLP-1 preparations, your clinic should have a receiving protocol that includes:
Inspect the packaging: Look for signs of temperature excursion β melted ice packs, condensation inside the box, activated TTI indicators, or damaged packaging. Document what you observe.
Check the temperature monitor: If a data logger or TTI is included, review it before accepting the shipment. If the monitor indicates a temperature excursion, do not use the preparation until you have consulted with the pharmacy.
Verify the BUD: Confirm the BUD on each vial is within an acceptable range for your intended use. A preparation that arrives with only a few days remaining on its BUD may not be usable.
Document receipt: Record the date and time of receipt, the condition of the packaging, the temperature monitor reading, and the lot numbers and BUDs of the preparations received.
Refrigerate immediately: Transfer preparations to your clinic's refrigerator immediately upon receipt. Do not leave them at room temperature while you complete other tasks.
Questions to Ask Your Pharmacy Partner
When evaluating a pharmacy partner's cold-chain practices, ask:
- What packaging system do you use for cold-chain shipments? Is it validated? For what transit time and ambient temperature range?
- Do you include temperature monitors in shipments? What type (data logger, TTI)?
- What carrier do you use? Do they have a pharmaceutical shipping program?
- What days do you ship? Do you avoid Friday shipments to prevent weekend delays?
- What is your protocol if a shipment experiences a temperature excursion? Will you replace the preparation?
- Can you provide documentation of your packaging validation?
A pharmacy partner that cannot answer these questions clearly has not invested adequately in cold-chain management. That is a risk for your clinic and your patients.
What Happens If the Cold Chain Is Broken?
If you receive a shipment that shows signs of a temperature excursion:
- Do not use the preparation until you have assessed the situation
- Contact the pharmacy immediately and provide details of the excursion (temperature reached, duration if known, condition of packaging)
- Document everything β photographs of the packaging, temperature monitor readings, and your communications with the pharmacy
- Follow the pharmacy's guidance on whether the preparation can be used or should be discarded
- File a complaint with the pharmacy and, if appropriate, with your state board of pharmacy or the FDA
